If it is for headaches then call your insurance to check their Medical Policy Manual. They should be able to give a detailed list of prerequisites to have insurance cover it. Most companies have them online and will walk you step by step to the policy so you can bookmark it and basically have your doctor do the checklist to ensure approval. Make sure he does a pre-determination, it ensures the insurance has all it need to cover it too.

Botox is poison that paralyzes the injected area. That's all it is. You may lose some wrinkles at the cost of also being unable to utilize that area partially or completely. I have a handful of friends who get it and I don't understand it at all.
