Happens all the time. Its usually the worst when its picked up by a fat offensive linemen who runs for 35 yards

The fumbling player can always advance the ball after recovering. What you're probably thinking of is that any other offensive player cannot within the two-minute warning of either half (this rule however isn't programmed into the sim... perhaps I should).

Not once has my defensive run priority been higher than 47 (for one game) this season.

Maybe it's just me, but that collection of defensive players strike me as not a bottom 3 unit in the league. 

If you can't stop the pass, you can't win in this league, period.

Won't bother about the offensive woes and how apparently things like 3 years of consistently good/great is outweighed by 13 games in the last season or other **** like that, but yeah. If the RNG gods and personal opinion ratings declare my defensive players incapable of stopping the pass, then so be it.
