No. No it doesn't.

A miscommunication is almost always somebody's fault.

If a QB reads coverage A and a WR reads coverage B, and it was coverage A, that's absolutely the WRs fault.

Went would it be different for the QB?

That was the equivalent of a turkey shot against a C2. NFL receivers are absolutely taught to throttle down in those instances. 

That goes against everything I was ever taught about route running. Could be, but I don't think it's as absolute as you make it sound. Sight lines are totally different for a QB when you're analyzing a receiver traveling down a horizontal plan vs vertical, plus he can use the sidelines to uncover you even with a field safety incoming.
