14 sales managers/directors/VPs on a conference call together for a "Lessons Learned" meeting (see: who to blame) to figure out why we lost a $1M+ project. Kill me now.

Lost it as in:

A) put a bid out and just didn't get it
Or
B) Had it and got kicked off

?  Either one is painful, but the first seems especially unnecessary.

We thought we were spec'd in as the preferred vendor by the end user, but we ended up losing the bid to a competitor. This was a copy of a previous project that we won, so it was a surprise that we lost.

So any company that services other companies runs into this time and again.  Those meetings are unnecessary.  The only real way to fix the issue is to ask the customer why he went the other way.

Agreed. And one of the issues is that when a company feels like they are spec'd in, they sometimes don't give the full-on service they should be
Meanwhile Competitor X is talking to the customer about what they like/don't like and honing in on a way to displace the incumbent
No clue on the specifics here - but I've been on both sides of that issue.

And sometimes you find out the customer switched because the sister of the CEO works at the competitor - nothing you can do about that
